What did this person do/break?:
Would this really effect the server/staff?:
Give Evidence?(this will make your report more likely to work):
Name of Reporter(your username):
Are there any witnesses?If so, how many and who? (more liability helps):
(This is for staff to edit as pleased) STAFF: Did this Report go through or no:
